How Can I make an appointment?

You can book an appointment using our online booking system, Jane, by clicking the "Book Online" button at the top right corner of our website. Alternatively, you can email us at hello@kollectivewellness.ca or call and leave a message at (587) 341-5157. If you choose to call or email, one of our team members will get back to you within 24 hours on business days.

Where are you located?

We are located in the Troika Business Centre at 41 Broadway Blvd, Sherwood Park, Alberta. Some of our neighbors include Confetti Sweets and Funky Petals Flowers. When you arrive, head to the far north end of the parking lot towards Costco and look for the business sign on the building that says Delta North (Entrance 22). Use that entrance and go up the stairs. Your appointment will be in either room 226 or 227, which is located through the last door before the emergency exit at the far end of the hallway.

Do you accept my health benefit insurance for Direct Billing?

We accept most health benefit insurance providers, provided your plan is set up for direct billing. To confirm if your plan is eligible for direct billing, please contact your health insurance company or your plan administrator through your workplace.

What information do you need if I am listed on my family members health benefit insurance?

If you are listed on a family member's health benefit insurance, we will need several key pieces of information to process your claims efficiently. Please provide the following details:

  • Insurance Policyholder's Information: This includes the full name, date of birth, and contact information of the primary policyholder (the family member who holds the insurance policy).

  • Insurance Provider Details: The name of the insurance company, the policy number, and the group number if applicable.

  • Your Information: Your full name, date of birth, and relationship to the policyholder.

  • Insurance Card: A copy of the insurance card, both front and back, to ensure we have all necessary information including any relevant identification numbers.

  • Authorization if Required: Some insurance policies may require authorization or a signed consent from the policyholder to process claims on behalf of a dependent. This usually is processed through the Insurance company - contact your plan administrator for this paperwork.

Can I direct bill my health benefit insurance for my Manual Osteopathy Treatment?

If your health benefit insurance is with a different provider, you can still receive coverage by submitting the receipt to your insurance company, provided that your plan covers manual osteopathy.

Please check with your health benefit provider to confirm the specifics of your coverage. Currently, Canada Life does not offer coverage of MOT claims. If you need any assistance with the billing process or have questions about your insurance, feel free to contact our office.

What paperwork do I need to fill out before my appointment?

Prior to your first appointment, you will receive an email containing a link to your online Health History form and Consent to Treat form. It is essential that these forms are completed at least 24 hours before your appointment to ensure we have sufficient time to review your information and prepare for your visit.

For each follow-up appointment, you will receive an email with a Consent for Sensitive Areas form (if applicable) and a Consent to Treat form. These forms must be completed before each session to ensure we have your updated consent and can provide the best possible care.

In addition to these forms, please bring any relevant medical records or documents that might help us understand your health history better. This includes previous treatment records, diagnostic reports, or any other documentation that could assist in providing comprehensive care.

Completing these forms accurately and promptly helps us tailor our treatments to your specific needs and ensures a smooth and efficient appointment. If you have any questions or need assistance with the forms, please do not hesitate to contact us.

Why do I need to fill out my peperwork before my appointment?

Filling out a health history and consent forms before your appointment is essential for several reasons. Firstly, it allows our practitioners to create a personalized treatment plan tailored specifically to your needs by providing vital information about your medical background, current health conditions, and previous treatments. Understanding your health history helps us identify any potential risks or contraindications for certain treatments, ensuring your safety. Additionally, the consent forms ensure you are fully informed about the treatments you will receive, outlining the benefits, risks, and alternatives for educated decision-making. This process is also a legal requirement that protects both you and our practitioners. Finally, having a comprehensive health history on file allows us to monitor your progress and make necessary adjustments to your treatment plan over time, ensuring continuity of care. At Kollectiv Health, we are dedicated to delivering safe, effective, and personalized treatment for your unique health needs, and completing these forms accurately helps us achieve this goal.

How often should I book my appointments?

Your health care provider will sit down with you prior to your first appointment to discuss your treatment goals and expected outcomes. Based on this initial assessment and the outcome of your first appointment, your provider will recommend when you should book your next appointment.

Once you have reached the maintenance phase of your treatment plan, we generally recommend booking appointments every 4 to 6 weeks, depending on your individual needs. However, some people may not require a maintenance phase and will be advised to re-book as symptoms start to arise.

To ensure you get your preferred appointment times, we encourage you to book your appointments ahead of time, as our schedule tends to fill up quickly. This proactive approach helps maintain consistency in your treatment and ensures you receive the care you need without unnecessary delays.

What if I can't find a time available that works for my schedule?

If you can't find an available time that fits your schedule, don't worry—we have several options to accommodate you. We maintain a waitlist, and you can add yourself to it directly through our online booking site on the Dashboard. You will be able to set your request for a specific day and time or for a range of time - and the system will show you a list of your requests on your Dashboard as well.. This ensures you will be contacted immediately if an appointment becomes available that matches your preferred times.

Additionally, if you indicate that you are looking to book an appointment but can't find a suitable time, please let us know. We may be able to adjust our schedule to better accommodate your needs.

What forms of payment do you accept?

We accept Credit Card (Visa, Mastercard), debit, cash and cheque. 

If I need a copy of my reciept, how can I get one?

 

You can access your reciepts online from the booking page dashboard. When you sign-in to the online booking platform Jane, the first page it takes you to is the dashboard. On the Left-hand options menu, there is a tab (about half-way down) labelled "Receipts". Once in this tab, it will list all your reciepts and give you the option to download a copy to keep for your own records.

What is the difference between acupuncture and dry needling?

While both acupuncture and dry needling involve the use of thin needles, they are based on different philosophies and approaches:
